Currently, the work book is the main document containing information about labor activity and experience of the individual.

Valid forms of work books

The form of the work book, as well as the Rules for its execution, storage, production of forms and supplying employers with them, were approved by Decree of the Government of the Russian Federation of April 16, 2003 No. 225.

Along with the form given in the above normative act and applied from 01/01/2004, work books approved by the following decrees are also valid in the territory of the Russian Federation:

  • Council of People's Commissars of the USSR of December 20, 1938 No. 1320;
  • All-Union Central Council of Trade Unions, Council of Ministers of the USSR dated 09/06/1973 No. 656;
  • Council of Ministers of the USSR dated April 21, 1975 No. 310 (labor books of collective farmers).

Thus, if a document is issued to an employee for the first time, a form approved by Resolution No. 225 is used. At the same time, labor. employee books started before 01/01/2004 do not require replacement, but continue to be used in the usual manner.

Who should keep work books

According to Art. 66 of the Labor Code of the Russian Federation, each employer, except for an individual who does not have the status of an individual entrepreneur, is obliged to keep work books for each employee who has worked for him for more than 5 days, provided that given place work is essential for the employee.

In this way, individual entrepreneurs must fill out work books for their hired personnel in the general manner.

Records are entered in the labor official organization (IP), authorized by order of the head (IP). As a rule, such a person is an employee of the personnel department.

If the official responsible for maintaining work books, not assigned, this function is performed by the head of the organization (IP).

Making work. books for an individual employed for the first time are produced by the employer no later than 7 days from the date of employment in the presence of the employee.

Note: The individual entrepreneur does not keep a work book for himself. It is obvious that an entrepreneur cannot conclude an employment contract with himself, that is, in accordance with Art. 20 of the Labor Code of the Russian Federation, by an employee, as one of the parties labor relations, he is not. And work books are maintained only in relation to employees (Article 66 of the Labor Code of the Russian Federation).

What records are entered in the labor

The book should include the following information:

  • about the employee;
  • about the work they do;
  • about layoffs;
  • grounds for termination employment contract;
  • transfer to another permanent job;
  • about the awards.

Note: information about disciplinary action are entered in the work book only if the result of such a penalty is dismissal.

At the initiative of the employee, data on part-time work can be entered into the labor work: both external and internal (see sample below).

To make an entry on external part-time work, an employee must submit a document at the main place of work confirming additional labor activity with another employer. A certificate from the place of work, as well as certified copies of an order or an employment contract, can serve as such a document.

With an internal combination, the employee is not required to provide additional documents.

Note: if individual performs work under a civil law contract (work contract), no entries are made in the work book. Relations between the parties arising under the GPC agreement are regulated by the Civil Code of the Russian Federation and are not included in the scope of labor legislation.

General requirements for filling

Instructions for the execution of work books were approved by the Decree of the Ministry of Labor of October 10, 2003 No. 69 (hereinafter referred to as the Instruction).

1. Dates in all sections of the work. books are written in Arabic numerals in the format DD.MM.YYYY.

Note: despite the requirement of the Instruction, on the title page of the work book for the date of filling, a line is highlighted that provides for entering the month in words. In this case, experts recommend writing the date as suggested in the form, i.e. indicate the month in words, and the date and year in Arabic numerals.

2. It is allowed to use ballpoint, gel, fountain pens in black, blue, purple. You can use pens with light-resistant ink.

3. You can not use abbreviations: "Ex." instead of "Order", "Disp." instead of "Decree", "TK" instead of "Labor Code", etc.

4. In sections containing information about work and awards, it is strictly forbidden to cross out inaccurate, erroneous or other invalid entries.

Fill in information about the employee

1. Full name is indicated in full, without replacing the name and patronymic with initials.

2. The date of birth is recorded on the basis of a passport or other identity document (foreign passport, driver's license, etc.).

3. Information about education is indicated as follows:

  • basic general (9 classes);
  • secondary general (11 classes);
  • secondary vocational (technical school, college, etc.);
  • higher professional (university);
  • postgraduate professional (postgraduate studies).

Note: with the entry into force of the law dated December 29, 2012 No. 273-FZ, the level of education "primary vocational" contained in the Instruction is no longer applicable.

Also, on the basis of the above Federal Law, the level of education "higher professional" is applied to bachelor's, specialist's and master's degrees.

An entry on education is made only on the basis of properly certified documentation (certificate, diploma, certificate, etc.). Information about incomplete education can be made on the basis of a student card, grade book, certificate from educational institution etc.

4. The profession (specialty) is also indicated on the basis of the document on education.

Note: diplomas of graduation from the university often indicate the field of activity as a specialty (for example: "Economics and management at the enterprise"), therefore it is better to fill in this line based on the information about the award of the qualification (for example: "Economist").

5. The date of issuance of the book is filled in. Then the employee certifies the entered information with his signature.

Note: in the form of the work book under the line intended for signature responsible person, written "clearly". Normative acts of labor legislation do not specify this concept. Therefore, experts recommend that an official not put a personal signature, but write down his last name clearly and legibly in this line.

At the same time, no responsibility is provided for affixing a personal signature, for deciphering the surname and initials, or using other possible options for filling out this line.

If the organization (IP) does not have a round seal, Rostrud also allows the use of the internal seal of the personnel service, provided that the employer can be clearly identified from the imprint (name of organization or full name of the entrepreneur, TIN, OGRN or OGRIP).

7. Changes in the records of the last name, first name and patronymic, as well as the date of birth are made on the basis of: passport, birth certificates, marriage or divorce, change of name - with the obligatory indication of the details of these documents.

The above changes are made in the following order:

  • one line crosses out the information to be corrected;
  • new data is written;
  • a link to the relevant document is indicated on the inside cover of the work book and is certified by the signature of the responsible person and the seal of the employer (employer's personnel department).

8. Changes in data on education, specialty, profession are made by supplementing the information entered earlier. You don't need to delete anything. A link to the document on the basis of which the additions were made is not indicated on the cover of the work book.

Filling out job details

1. In gr. 3 as a title, the full and abbreviated (if any) name of the organization (IE) is indicated.

4. In gr. 3 a record is made of acceptance (appointment) to a specific structural subdivision legal entity(IP), if the condition of work in this unit is reflected in labor agreement, and the name of the position (work, specialty, profession, indicating the rank, class, category) in accordance with.

In cases where working conditions imply compensation, benefits or restrictions, the name of the specialty (positions, jobs, etc.) is indicated in accordance with the qualification reference books.

5. In gr. 4, the name and details of the document according to which the employee was hired (order, instruction, etc.) are recorded.

Note: a record of employment on the basis of a fixed-term employment contract is completely similar to a record of employment under an indefinite employment contract.

Note: if the employment record is made in respect of CEO organizations, then the wording “Appointed” (“Elected”) is used, and in gr. 4 details of the protocol are indicated general meeting founders (decisions of the sole founder, order on the appointment of the general director).

Information about work in respect of which entries are required in the work book also include:

  • assigning a new category (category, class) to an employee;
  • establishing a second and subsequent specialty for an employee;
  • changes (additions) in the staffing of the organization (IP);
  • transfer to another permanent job;
  • renaming an organization or changing its legal form;
  • restoration of uninterrupted work experience in the prescribed manner (for example: based on the decision of the commission on labor disputes);
  • non-inclusion in the continuous work experience of the period of serving a sentence in the form of correctional labor;
  • military service, as well as service in the internal affairs bodies, tax police, customs, penitentiary system, etc.;
  • training in advanced training courses, retraining.

Filling in the information about the award

The employee's work book contains information about the following types of incentives:

  • honorary titles, awards for services to the state;
  • diplomas, certificates of honor, badges;
  • other types of incentives in accordance with the current legislation and internal documents of the company (for example: a collective agreement).

Note: information about bonuses on wages is not recorded in the work book. But this does not apply to one-time and unscheduled bonuses for particularly distinguished employees - such a bonus can be entered in the book.

The procedure for entering information about the award is identical to the procedure for filling in data about work:

  1. In gr. 3 as a title, the full and abbreviated (if any) name of the employer is indicated.
  2. Under this heading in 1 indicates the serial number of the record.
  3. In gr. 2 enter the date of the award.
  4. In gr. 3 indicates the type of remuneration, the merits of the recipient and the organization issuing the award.
  5. In gr. 4 name and details of the document according to which the award is made.

Filling out information about the dismissal

Resignations are made in the following order:

1) In gr. 1 indicates the serial number of the record.

2) In gr. 2 is the date of dismissal (termination of the employment contract).

In this case, the date of dismissal is considered the last day of work, unless otherwise provided by an agreement between the employee and the employer, the employment contract or the current legislation.

3) In gr. 3, a record is made of the reason for dismissal.

Note: according to Art. 84.1 of the Labor Code of the Russian Federation, an entry in the employee's work book about dismissal is made in strict accordance with the wording of the labor code or other federal law. In this case, it is required to indicate not only the paragraph and article of the Labor Code of the Russian Federation, but also part of it.

Note that the parts in the codex are not numbered. To find out the part number, you need to count the paragraphs in the paragraph of the article in order: the first paragraph (even if it is only one) is the first part, the second paragraph is the second part, etc.

For example: in paragraph 1 of Art. 77 is just one line, we write “clause 1 part one article 77”. The paragraph and number of the article of the Labor Code are indicated in Arabic numerals, and the part number is written in words.

When terminating an employment contract at the request of an employee for reasons for which the law provides for benefits or other benefits, a record of dismissal is made indicating such reasons (the need to care for a child under 14 years old, transferring her husband to work in another area, etc.).

4) In gr. 4 indicates the name and details of the document on the basis of which the record of dismissal is made.

Under the record of dismissal, the employee puts his personal signature, confirming the fact of familiarization with the specified information (except when the work is sent to the dismissed employee by mail).

At the same time, it is not necessary to add additional wording to the document (“Acquainted”, “Record is correct”, “I confirm the information”, etc.).

Note: other than the general grounds provided for labor law, the dismissal of the head of a legal entity may be caused by additional grounds set forth in Art. 278 of the Labor Code of the Russian Federation (for example: bankruptcy of an organization, improper performance by the head of his duties, decision-making by the owner of the property of the organization, other reasons).

Acquisition of work forms. books

The procedure for providing employers with forms of work books and to them was approved by order of the Ministry of Finance of the Russian Federation of December 22, 2003 No. 117n.

These forms are produced by the GOZNAK Association and are sold both through the Association itself and through intermediaries who have concluded supply contracts with it. Official suppliers, both wholesale and retail, are required to present a copy of the agreement with GOZNAK (or a copy of the agreement with a wholesaler selling GOZNAK products) at the request of the buyer.

The purchase of forms is the responsibility of the employer; forcing an employee to purchase a form of a work book or an insert in it on their own is prohibited.

At the same time, the organization (IP) has the right, when filling out the form, to demand reimbursement from the employee, except in cases where it was necessary to start a new labor or insert due to the fault of the employer: incorrect execution, loss of documents due to force majeure.

Note: reimbursement of costs for the purchase of forms is provided for by Decree No. 225, but the employer may refuse to charge employees, for this this position must be fixed in local act enterprises (for example: in the Labor Regulations).

In order not to violate the established deadlines for making entries in the work books of employees, employers are recommended to have in stock at least one copy of the work form and an insert to it.

Accounting and storage of work books

The organization (IP) keeps records of new copies of labor and inserts for them in a special one.

Responsible personnel officer receives forms in the accounting department on the basis of a written application, and at the end of the month reports for the forms used and the amounts received from employees.

In addition, the company is required to conduct, in which information is entered not only about new forms, but also about all work. books kept by the employer.

Employment books, as well as inserts to them, are strict reporting forms, therefore they must be stored in safes, metal cabinets or in other conditions that guarantee the safety of documents.

Books, inserts and, not received by employees upon dismissal, must be stored in the organization (IP) on demand, and unclaimed - for 75 years.

Note: the law does not prohibit issuing work books into the hands of employees, but the employer will be responsible for their safety.

Issuance of a work book upon dismissal

The work book is issued to the employee on the day the employment contract is terminated. If for some reason this is impossible to do (the employee did not appear to receive the book, refuses to receive it, etc.), the employer should send a written letter to the citizen about the need to appear for the document or agree to send the book by mail.

Thus, the employer will relieve himself of responsibility for the untimely issuance of labor.

Sometimes it is impossible to issue a work book to an employee on the day of dismissal. As a rule, this situation occurs when the day of dismissal does not coincide with the last working day: when an employee who is in maternity leave; upon dismissal of a truant who is absent from the workplace.

Any organization certainly needs to have work books for everyone who works in it on an ongoing basis and for more than five days. It is allowed to make marks on the pages of the book with a blue pen (purple as an option) or black, the use of various kinds of abbreviations is considered a mistake.

Title page information:

  1. Name. It must be written in full form, as in the passport, without the use of abbreviated forms and initials. Data can be found in a passport, driving license, military ID and similar documents.
  2. Date of Birth. Arabic numerals are used to write it. When specifying the day and month, two ordinal numbers are used, and the year is written as four ordinal.
  3. Education. This line contains the level of education available, which can be secondary vocational, higher or other. Here you can enter information about education that is not completed at the moment. The data necessary for the correct completion of this paragraph should be taken from the documents provided by the employee.
  4. Speciality. Here the main working specialty of the owner of the book, received by him in an educational institution, fits in. This information is also specified according to the documents on education provided.
  5. Date of completion. As a rule, an important date is fixed here - the day the first entry in the book was made. Feature specifying the date in this line - the month can be written in words, and this will not be considered an error.
  6. Signature of the owner of the book. Everything is clear here, the job seeker himself signs.
  7. The signature of the employee who deals with all issues related to the execution of such documents. There are options here. Ideally, the personnel department should be in charge of such things, respectively, the signatures in the books are made by his boss. However, not every company has its own personnel department, so often such responsibility is assigned to an accountant, or even to a manager.
  8. Seal. It confirms everything that was stated earlier. This may be the official seal of the company or its personnel department, if any.

Entering data about the main work

  1. Company name. It is written in the third column of the table, first in full, without abbreviations such as OOO, ZAO, etc., and then the abbreviated form of the name is additionally signed in brackets.
  2. Record number. The serial number of absolutely any job record is entered in the required column of the table, i.e., for example, at the first employment it will be the number “1”, when transferring it will be “2”, and so on. It is noteworthy that this figure is placed on the line following the name of the company, and the name itself forms a kind of "cap" over all subsequent marks.
  3. The date. The date of official registration for work is entered in the columns of the table specially designed for this purpose in Arabic numerals. As for the line, the information is located on the same line as the serial number of this entry.
  4. Job application record. It is written in the third column and begins on the same line that contains the date of the device for work. The name of the division of the company where the employee is accepted, and his position are indicated.
  5. Indication of the document that became the basis for the employment of the owner of the book. Usually they get an order to hire a new employee. This information is entered in the last column on the same line as the previous entry.

Entering information about part-time work

Sometimes it happens that an employee performs certain duties, combining them with his main job. Information about such a labor initiative is recorded only at the request of the employee himself. If he has a desire and he asked to make a mark in the book, then the procedure for entering data will be similar to that described earlier, only the name of the organization does not need to be written a second time.

Transfer to another unit or change of position

It happens that an employee is transferred for further work to another department or change position. Naturally, such changes in his career path must be recorded in the labor.

  1. Record number. A new milestone in labor is marked, of course, with a new number in a row.
  2. The date. The date of the official translation is written in Arabic numerals.
  3. A record of a transfer or a new position. In the third column, the name of the unit where the owner of the labor is transferred and his new position are written.
  4. Basis for translation. The last column of the table contains the data of the transfer order.

Entering information about the dismissal

The dismissal of an employee is a reason for another entry into the labor force, and it must be done on time, since on the final working day it must be returned to the owner. Delay in such a matter may threaten with a fine and compensation for an employee who has stopped working in this organization.

  1. Serial number. Dismissal is another entry, and it must have its number in the column intended for this.
  2. The date. The date of the final working day is indicated here - it is she who is considered the date of the final dismissal. Naturally, the numbers must be Arabic.
  3. The reasons. In the third column, where the position and place of work are recorded, you need to make a note about the reason for dismissal, not forgetting to refer to the required article of the Labor Code. For example, if the dismissal was preceded by the immediate desire of the employee, one must refer to Article 77, paragraph 3.
  4. Base. The last column contains the data of that document (usually an order for dismissal), on the basis of which the dismissal occurred.
  5. Seal. After completing all the records, you need to seal them, as well as the signature of the head of the company or other authorized person. The resigned employee also signs, and the dismissal can be considered completed.

Making corrections

Sometimes errors occur when writing down various information in the worksheet, for example, instead of "senior engineer" it is simply written "engineer". The error, of course, needs to be corrected, we have already written a more detailed article about. It is forbidden to cross out and gloss over anything, and there is only one way to correct it - to invalidate the erroneously indicated data:

  1. Number . The number of the next record is put down.
  2. The date. The date from which the erroneous entry will be considered invalid is recorded.
  3. In the third column, the following text is written "Entry under No. _ is invalid."
  4. A new record is made with up-to-date and correct information, for which its serial number and date are also affixed.

The work book stores all the information about the work activity of the employee and is used to determine the length of his experience. The current forms of the work book and insert are established in the Post. No. 225 of April 16, 2003 How to fill out a work book correctly is indicated in the Post. No. 69 of October 10, 2003. These resolutions contain discrepancies on some issues, which requires recourse to the Labor Code and makes it even more difficult to draw up a work book.

This is especially true for private entrepreneurs who are forced to take on the responsibility of running personnel records. The slightest deviation from the standards for filling out a work book leads to the invalidity of the information entered, as well as various errors made in the records themselves. To eliminate the consequences, corrections are made in the corresponding columns strictly according to the established pattern. In exceptional cases, the employee is issued a duplicate.

When does it open

Each work book has its own number, is issued to the employee for life and cannot be replaced, except in some cases. If a recently hired employee does not have this document, you need to open a new one for him. If there was one, but for some reason cannot be provided, this information is reflected in the act, which is certified by signatures: the person issuing the new work book, the employee and the witness.

The document is created for an employee who has worked in the company for at least six days, and must be issued within a week from the date of his employment. This procedure is carried out in the presence of an employee, from whose wages the cost of the form is subsequently deducted.

How to fill

All entries in the work book are entered by an official of the organization authorized to do so by order of the head, most often by an employee of the personnel department. In his absence, this may be an accountant or director of the enterprise. The document is subject to registration in the ledger and is kept by the employer until the employee is dismissed or transferred to another place of work. If necessary, the employee is issued an extract from the book at the place of demand.

Records are kept in legible handwriting, all words are given in full, without any abbreviations. The numerical expression of dates, characteristics and other information is indicated only in Arabic numerals. Also, you can not write the name of the month in words. The correct date format is 03/05/2020. It is acceptable to use ballpoint, gel and fountain pens. The paste, gel and ink contained in them must have increased resistance to water and ultraviolet rays.   • register of work books;

Please note that the rules for filling out work books oblige you to make adjustments in case of a possible change:

  • company name (incl. legal form);
  • type of printing (using different ones in one book is unacceptable!).

On the first page of the work book, you must specify all the information about the employee, guided by the documents provided by him. It must contain:

  • F&O of the owner;
  • date of birth (in numerical terms);
  • level of education (secondary, secondary specialized, higher);
  • the name of the profession indicated in the diploma;
  • date of opening of the work book;
  • employee's signature;
  • signature of the person filling out the document;
  • official seal of the organization.

Each page of the middle and most extensive section consists of four columns containing:

  1. Sequence number of the record (starting from #1 and onwards).
  2. Date of employment and other events.
  3. The record itself, which is information about the organization (its full and abbreviated name), job title (with qualifications) and department. In addition to the fact of being hired, cases of transfer to another position and dismissal are also recorded here. Information about this must be entered in the column within 7 days from the date of issuance of the order, with the exception of the dismissal record, which is drawn up directly on the day it is made.
  4. Documentary confirmation of the entry - the name of the order, the date of its issue and the number (in that order).

additional information

  1. At the request of the employee, information about part-time work is entered into it. The entry is made on the basis of documents provided by another enterprise where the employee is employed in a combined position. In this case, the same principles for filling out the column are used as when entering information about the main work. The work book should be kept by an employee of the personnel department at the place of main work, who is the only one who has the right to enter any information into it.
  2. The acquisition of a second profession, obtaining a rank, advanced training are also reflected in this section.
  3. If there is a renaming of the enterprise, an appropriate entry is made indicating the document that served as the basis for this. This information will be able to explain the discrepancy in the seals affixed during the employment and dismissal of an employee. The same actions are performed when renaming a position.
  4. In case of reorganization of the enterprise, the section notes the dismissal of the employee with his subsequent admission to the new company.

A. S. Kopacheva joined the position of legal adviser at Avelan OJSC on February 27, 2016. On August 12, 2016, she was transferred to the position of senior legal adviser. This fact was reflected in the section "Information about the work".

When an employee is dismissed in column No. 3, it is necessary to indicate the normative act of the Labor Code, according to which this measure was applied. Awards

This section contains information about the titles assigned to the employee, diplomas, certificates and other awards provided for by internal regulations companies and legislation. These include verbal gratitude from management, as well as regularly paid cash bonuses included in the payroll system. Which explains the complete lack of information in this section, found in the work books of most workers. Moreover, various certificates and diplomas issued to an employee without issuing an appropriate order are also only a nominal expression of gratitude, not reflected either in the work book or in the personal file.

Insert design

If all the free places in the work book have been used up and there is nowhere to make new entries, the insert opens. Usually it is sewn into the end of the document, since there are no specific instructions for this in the instructions for filling out work books. On the title page (or cover) of the main book, a stamp is placed on the issuance of the insert, indicating its series and number.

The rules for maintaining both documents are identical, the title page of the insert is also to be filled out. The numbering of entries continues after the number at which the completion of the work book ended. In the same manner, the record of dismissal, transfer to another permanent job is invalidated if the dismissal or transfer is recognized as illegal by the employer, the control and supervisory authority, the review body labor disputes or by the court and reinstatement in the previous job or change the wording of the reason for dismissal. For example: "The entry for the number such and such is invalid, restored to the previous job." When the wording of the reason for dismissal is changed, an entry is made: "The entry for the number such and such is invalid, dismissed (the new wording is indicated)". In column 4, a reference is made to the order (instruction) or other decision of the employer to reinstate at work or change the wording of the reason for dismissal.

If there is an entry in the work book about dismissal or transfer to another permanent job, subsequently recognized as invalid, a duplicate of the work book is issued upon a written application of the employee without making an entry declared invalid. At the same time, in the upper right corner of the first page of the duplicate work book, an inscription is made: "Duplicate". On the first page (title page) of the previous work book, it is written: "A duplicate was issued instead" indicating its series and number.

2. Filling in information about the employee

2.1. The information about the employee provided for by the Rules for maintaining work books, indicated on the first page (title page) of work books, is filled in as follows:

surname, name and patronymic are indicated in full, without abbreviation or replacement of the name and patronymic with initials, the date of birth is recorded in full (day, month, year) on the basis of a passport or other identification document (for example, a military ID, foreign passport, driver's license, etc. .);

a record of education (basic general, secondary general, primary vocational, secondary vocational, higher professional and postgraduate professional) is carried out only on the basis of properly certified documents (certificate, certificate, diploma, etc.);

a record of incomplete education of the appropriate level can be made on the basis of the submitted duly certified documents (student card, grade book, certificate of an educational institution, etc.);

the profession and/or specialty are indicated on the basis of documents on education, qualifications or the availability of special knowledge (when applying for a job requiring special knowledge or special training) or other properly executed documents.

2.2. After indicating the date of filling out the work book, the employee certifies with his signature on the first page (title page) of the work book that the information entered is correct.

The first page (title page) of the work book is also signed by the person responsible for issuing work books, after which the seal of the organization (stamp of the personnel service) (if any) is affixed, in which the work book was first filled out.

2.3. Changes in records in work books about last name, first name, patronymic and date of birth are made on the basis of a passport, birth certificates, marriage certificates, divorce certificates, changes in last name, first name, patronymic and other documents and with reference to their number and date.

These changes are made on the first page (title page) of the work book. The former surname or first name, patronymic, date of birth is crossed out with one line and new data is recorded. Links to the relevant documents are made on the inside cover of the work book and are certified by the signature of the employer or a person specially authorized by him and the seal of the organization (or the seal of the personnel service) (if there is a seal).

2.4. Change (addition) on the first page (title page) of the work book of records of new education, profession, specialty received is carried out by supplementing the existing records (if they already exist) or filling in the corresponding lines without crossing out the previously made records.

3. Filling in job details

3.1. In column 3 of the section "Information about work" of the work book, the full name of the organization, as well as the abbreviated name of the organization (if any) are indicated in the form of a heading.

Under this heading in column 1 the serial number of the entry being made is put, in column 2 the date of employment is indicated.

In column 3, an entry is made about acceptance or appointment to a structural unit of the organization indicating its specific name (if the condition for working in a specific structural unit is included in the employment contract as essential), the name of the position (job), specialty, profession, indicating qualifications, and column 4 contains the date and number of the order (instruction) or other decision of the employer, according to which the employee was hired. Records about the title of the position (job), specialty, profession, indicating qualifications are made, as a rule, in accordance with the organization's staffing table. If, in accordance with federal laws, the provision of benefits or restrictions is associated with the performance of work in certain positions, specialties or professions, then the names of these positions, specialties or professions and qualification requirements they must comply with the names and requirements provided for by the relevant qualification reference books.

Amendments and additions made in accordance with the established procedure in qualification guides, staffing organizations are brought to the attention of employees, after which appropriate changes and additions are made to their work books on the basis of an order (instruction) or other decision of the employer.

If an employee is assigned a new category (class, category, etc.) during the period of work, then a corresponding entry is made in accordance with the established procedure.

The establishment of a second and subsequent profession, specialty or other qualification for an employee is noted in the work book indicating the categories, classes or other categories of these professions, specialties or skill levels. For example, the second profession "Electric and gas welder" was assigned to a repairman with the assignment of 3rd category. In this case, in the work book in column 1, column 2, the date of establishment of the second profession is indicated, in column 3 the entry is made: "The second profession "Electric and gas welder" was established with the assignment of 3 categories", column 4 indicates the corresponding certificate, its number and date.

At the request of the employee, information about part-time work is recorded in the work book at the place of the main job on the basis of a document confirming part-time work. In column 1 of the section "Information about the work" of the work book, the serial number of the entry is put, in column 2 the date of employment as a part-time worker is indicated, in column 3 an entry is made about the acceptance or appointment as a part-time worker in the structural unit of the organization indicating its specific name ( if the condition of working in a specific structural unit is included in the employment contract as an essential one), the name of the position, specialty, profession, indicating qualifications, column 4 indicates the name of the document on the basis of which the entry was made, with reference to its date and number. In the same order, a record of dismissal from this job is made.

3.2. If the name of the organization changes during the work of the employee, then a separate line in column 3 of the "Information about work" section of the work book is written about this: "Organization such and such was renamed from such and such a date to such and such", and in column 4 it is affixed the basis for renaming is an order (instruction) or other decision of the employer, its date and number.

3.3. In the work books of persons who have served a sentence in the form of corrective labor, an entry on the non-inclusion of the time of work during the period of serving the sentence in the continuous length of service is made as follows. In the section "Information about work" of the work book in column 1, the serial number of the entry is put, in column 2 - the date the entry was made, in column 3 the entry is made: "Working time from such and such a date (day, month, year) to such and such date (day, month, year) is not counted in the continuous length of service. Column 4 indicates the basis for making an entry in the work book - an order (instruction) or other decision of the employer (issued in accordance with the verdict (determination) of the court), its date and number.

3.4. When restoring the continuous work experience in accordance with the established procedure, an entry is made in the work book of the employee at the last place of work in column 3 of the section "Information about work": "The continuous work experience was restored from such and such a date, month, year", in column 4 a reference is made to the corresponding name of the document on the basis of which the entry was made, with reference to its date and number.

4. Filling in information about the award

The procedure for entering information about the award is as follows: in column 3 of the section "Information about the award" of the work book, the full name of the organization, as well as the abbreviated name of the organization (if any) are indicated in the form of a heading; below in column 1 is the serial number of the entry (numbering that increases throughout the entire period of the employee's labor activity); column 2 indicates the date of the award; column 3 records who awarded the employee, for what achievements and what award; column 4 indicates the name of the document on the basis of which the entry was made, with reference to its date and number.

5. Filling in information about the dismissal (termination of the employment contract)

5.1. An entry on dismissal (termination of an employment contract) in the employee's work book is made in the following order: in column 1, the serial number of the entry is put; column 2 indicates the date of dismissal (termination of the employment contract); in column 3, an entry is made about the reason for dismissal (termination of the employment contract); column 4 indicates the name of the document on the basis of which the entry was made - an order (instruction) or other decision of the employer, its date and number.

The date of dismissal (termination of the employment contract) is the last day of work, unless otherwise provided by federal law, the employment contract or an agreement between the employer and the employee.

For example, when an employment contract with an employee is terminated due to a reduction in the staff, October 10, 2003 is determined as the last day of his work. The following entry must be made in the work book of the employee: in column 1 of the section "Information about work" the serial number of the entry is put, in column 2 the date of dismissal is indicated (10.10.2003), in column 3 the entry is made: paragraph 2 of Article 81 of the Labor Code of the Russian Federation", column 4 indicates the date and number of the order (instruction) or other decision of the employer to dismiss.

5.2. Upon termination of an employment contract on the grounds provided for in Article 77 of the Labor Code of the Russian Federation (with the exception of cases of termination of an employment contract at the initiative of the employer and due to circumstances beyond the control of the parties (paragraphs 4 and this article), an entry is made in the work book about the dismissal (termination employment contract) with reference to the relevant paragraph of this article.

For example: "Dismissed by agreement of the parties, paragraph 1 of Article 77 of the Labor Code of the Russian Federation" or "Fired of his own free will, paragraph 3 of Article 77

5.3. When terminating an employment contract at the initiative of the employer, an entry is made in the work book about the dismissal (termination of the employment contract) with reference to the relevant paragraph of Article 81 of the Labor Code of the Russian Federation or other grounds for terminating the employment contract at the initiative of the employer, provided for by law.

For example: "Fired due to the liquidation of the organization, paragraph 1 of Article 81 of the Labor Code of the Russian Federation" or "Dismissed due to the termination of access to state secrets, paragraph 12 of Article 81 of the Labor Code of the Russian Federation."

5.4. Upon termination of an employment contract due to circumstances beyond the control of the parties, an entry is made in the work book on the grounds for termination of the employment contract with reference to the relevant paragraph of Article 83 of the Labor Code of the Russian Federation.

For example: "Fired due to non-election to the position, paragraph 3 of Article 83 of the Labor Code of the Russian Federation" or "Employment contract terminated due to the death of an employee, paragraph 6 of Article 83 of the Labor Code of the Russian Federation."

5.5. Upon termination of an employment contract on additional grounds provided for by the Labor Code of the Russian Federation or other federal laws, entries on the dismissal (termination of the employment contract) are made in the work book with reference to the relevant article of the Labor Code of the Russian Federation or other federal law.

For example: "Fired due to a repeated gross violation of the charter of an educational institution during the year, paragraph 1 of Article 336 of the Labor Code of the Russian Federation" or "Dismissed due to reaching the age limit established for filling a public position in the civil service, paragraph 2 (1) of Article 25 federal law dated July 31, 1995 N 119-FZ "On the fundamentals of the public service of the Russian Federation".

5.6. When terminating an employment contract at the initiative of an employee for reasons to which the law relates the provision of certain benefits and benefits, an entry on dismissal (termination of the employment contract) is made in the work book indicating these reasons. For example: "Fired of her own free will in connection with the transfer of her husband to work in another locality, paragraph 3 of Article 77 of the Labor Code of the Russian Federation" or "Fired of her own free will in connection with the need to care for a child under the age of 14, paragraph 3 of the article 77 of the Labor Code of the Russian Federation".

6. Features of filling in information about the dismissal (termination of the employment contract) and admission (appointment) to work in connection with the transfer of the employee to another permanent job to another employer (to another organization) or his transition to elective work(job title)

6.1. Upon dismissal (termination of the employment contract) in connection with the transfer of the employee to another permanent job to another employer (to another organization), column 3 of the section "Information about work" of the work book indicates in what order the transfer is carried out: at the request of the employee or with his consent .

Upon admission to a new place of work in the employee's work book in column 3 of the "Information about work" section, an entry is made as provided for in clause 3.1 of this Instruction, indicating that the employee was accepted (appointed) in the order of transfer.

6.2. Upon dismissal (termination of the employment contract) in connection with the transfer of the employee to an elective job (position) to another employer (to another organization), an entry is made in the work book: "Dismissed due to the transfer to an elective job (position) in (the name of the organization is indicated) , paragraph 5 of Article 77 of the Labor Code of the Russian Federation".

At the new place of work, after indicating the full name of the elected body, as well as the abbreviated name of the elected body (if any), in column 3 of the "Information about work" section of the work book, an entry is made about what job (position) the employee was elected to, and in column 4 indicates the decision of the elected body, the date and number of its adoption.

7. Features of filling out a duplicate work book

7.2. If an employee before entering this organization (to this employer) has already worked, then when filling out a duplicate work book in the section "Information about work" in column 3, first of all, an entry is made about the total and / or continuous work experience as an employee before entering this organization (to this employer), confirmed relevant documents.

The total work experience is recorded in total, that is, the total number of years, months, days of work is indicated without specifying which employer, in what periods of time and in what positions the owner of the work book worked in the past.

After that, the total and / or continuous work experience, confirmed by properly executed documents, is recorded for individual periods of work in the following order: column 2 indicates the date of employment; column 3 contains the name of the organization (employer) where the employee worked, as well as the structural unit and work (position), specialty, profession, indicating the qualification for which the employee was hired.

If the submitted documents confirm that the employee was transferred to another permanent job in the same organization (with the same employer), then a corresponding entry is also made about this.

Then, column 2 indicates the date of dismissal (termination of the employment contract), and column 3 - the reason (reason) for dismissal, if the document submitted by the employee contains such data.

In the event that the documents do not fully contain the above information about work in the past, only the information available in the documents is entered into the duplicate of the work book.

Column 4 indicates the name, date and number of the document on the basis of which the corresponding entries in the duplicate were made. Original documents confirming the length of service, after making copies of them and duly certifying them by the employer, or personnel service returned to their owner. The employer is obliged to assist the employee in obtaining documents confirming the length of service of his work prior to employment with this employer.

This Instruction, in accordance with clause 13 of the Rules for maintaining and storing work books, making blank work books and providing employers with them (hereinafter referred to as the Rules for maintaining work books), approved by Decree of the Government of the Russian Federation of April 16, 2003 N 225 "On work books", establishes the procedure for filling out work books, inserts in them, duplicates of work books (hereinafter referred to as work books).

1. General Provisions

1.1. Date entries in all sections of work books are made in Arabic numerals (day and month - two-digit, year - four-digit). For example, if an employee was hired on September 5, 2003, an entry is made in the work book: "09/05/2003".

Entries are made neatly, with a fountain or gel pen, rollerball pen (including ballpoint), light-resistant ink (paste, gel) in black, blue or purple and without any abbreviations. Upon termination of an employment contract on the grounds provided for in Article 77 of the Labor Code of the Russian Federation (with the exception of cases of termination of an employment contract at the initiative of the employer and due to circumstances beyond the control of the parties (paragraphs 4 and 10 of this article), an entry on dismissal is made in the work book ( termination of the employment contract) with reference to the relevant paragraph of this article.

For example: "Fired by agreement of the parties, clause 1" or "Fired of his own free will, clause 3 of Article 77 of the Labor Code of the Russian Federation".

5.3. When terminating an employment contract at the initiative of the employer, an entry is made in the work book about the dismissal (termination of the employment contract) with reference to the relevant paragraph of Article 81 of the Labor Code of the Russian Federation or other grounds for terminating the employment contract at the initiative of the employer, provided for by law.

For example: "Dismissed in connection with the liquidation of the organization, paragraph 1 of Article 81 of the Labor Code of the Russian Federation" or "Dismissed in connection with the termination of access to state secrets, paragraph 12 of Article 81 of the Labor Code of the Russian Federation".

5.4. Upon termination of the employment contract due to circumstances beyond the control of the parties, an entry is made in the work book on the grounds for termination of the employment contract with reference to the relevant paragraph.

For example: "Fired due to non-election to the position, paragraph 3 of Article 83 of the Labor Code of the Russian Federation" or "Employment contract terminated due to the death of the employee, paragraph 6 of Article 83 of the Labor Code of the Russian Federation".

5.5. Upon termination of an employment contract on additional grounds provided for by the Labor Code of the Russian Federation or other federal laws, entries on the dismissal (termination of the employment contract) are made in the work book with reference to the relevant article of the Labor Code of the Russian Federation or other federal law.

For example: "Dismissed due to a repeated gross violation of the charter of an educational institution during the year, paragraph 1 of Article 336 of the Labor Code of the Russian Federation" or "Dismissed due to reaching the age limit established for filling a public position in the civil service, paragraph 2 (1) of Article 25 of the Federal Law of July 31, 1995 N 119-FZ "On the Fundamentals of the Public Service of the Russian Federation".

5.6. When terminating an employment contract at the initiative of an employee for reasons to which the law relates the provision of certain benefits and benefits, an entry on dismissal (termination of the employment contract) is made in the work book indicating these reasons. For example: "Fired of her own free will in connection with the transfer of her husband to work in another locality, paragraph 3 of Article 77 of the Labor Code of the Russian Federation" or "Fired of her own free will in connection with the need to care for a child under the age of 14, paragraph 3 of the article 77 of the Labor Code of the Russian Federation".

6. Features of filling in information about the dismissal (termination of the employment contract) and admission (appointment) to work in connection with the transfer of the employee to another permanent job to another employer (to another organization) or his transfer to elective work (position)

6.1. Upon dismissal (termination of the employment contract) in connection with the transfer of the employee to another permanent job to another employer (to another organization), column 3 of the section "Information about work" of the work book indicates in what order the transfer is carried out: at the request of the employee or with his consent .

Upon admission to a new place of work in the employee's work book in column 3 of the "Information about work" section, an entry is made as provided for in clause 3.1 of this Instruction, indicating that the employee was accepted (appointed) in the order of transfer.

6.2. Upon dismissal (termination of the employment contract) in connection with the transfer of the employee to an elective job (position) to another employer (to another organization), an entry is made in the work book: "Dismissed due to the transfer to an elective job (position) in (the name of the organization is indicated) , paragraph 5 of Article 77 of the Labor Code of the Russian Federation ".

At the new place of work, after indicating the full name of the elected body, as well as the abbreviated name of the elected body (if any), in column 3 of the "Information about work" section of the work book, an entry is made about what job (position) the employee was elected to, and in column 4 indicates the decision of the elected body, the date and number of its adoption.

7. Features of filling out a duplicate work book

7.1. A duplicate of the work book is filled in in accordance with sections 1 - 6 of this Instruction.

7.2. If the employee has already worked before joining this organization (to this employer), then when filling out a duplicate work book in the section "Information about work" in column 3, first of all, an entry is made about the total and / or continuous work experience as an employee before entering the this organization (to this employer), confirmed by relevant documents.

The total work experience is recorded in total, that is, the total number of years, months, days of work is indicated without specifying which employer, in what periods of time and in what positions the owner of the work book worked in the past.

After that, the total and / or continuous work experience, confirmed by properly executed documents, is recorded for individual periods of work in the following order: column 2 indicates the date of employment; column 3 contains the name of the organization (employer) where the employee worked, as well as the structural unit and work (position), specialty, profession, indicating the qualification for which the employee was hired.

If the submitted documents confirm that the employee was transferred to another permanent job in the same organization (with the same employer), then a corresponding entry is also made about this.

Then, column 2 indicates the date of dismissal (termination of the employment contract), and column 3 - the reason (reason) for dismissal, if the document submitted by the employee contains such data.

In the event that the documents do not fully contain the above information about work in the past, only the information available in the documents is entered into the duplicate of the work book.

Column 4 indicates the name, date and number of the document on the basis of which the corresponding entries in the duplicate were made. The original documents confirming the length of service, after making copies of them and properly certifying them by the employer or the personnel department, are returned to their owner. The employer is obliged to assist the employee in obtaining documents confirming the length of service of his work prior to employment with this employer.
