IN OBSTETRIC HOSPITALS"
In connection with the release of new normative documents Ministry of Health of Russia, regulating sanitary and anti-epidemic rules,
I ORDER:
1. To make changes and additions to Appendix 1 "Instructions for the organization and implementation of preventive and anti-epidemic measures in obstetric hospitals" to the order of the Ministry of Health of Russia dated November 26, 1997 N 345 ​​"On improving measures for the prevention of nosocomial infections in obstetric hospitals" (Appendix).
2. The heads of the health authorities of the constituent entities of the Russian Federation and the chief doctors of the centers of state sanitary and epidemiological supervision in the constituent entities of the Russian Federation, the heads of healthcare institutions of federal subordination, accept the changes and additions approved by this order for management and execution.

CHANGES AND ADDITIONS TO
"INSTRUCTIONS FOR ORGANIZING AND CARRYING OUT
PREVENTIVE AND COUNTER-EPIDEMIC MEASURES IN
obstetric hospitals"
1. Clause 2.1. put in a new edition:
"2.1. Medical workers entering the maternity hospitals (departments) undergo medical checkup therapist, dermatovenereologist, dentist, otolaryngologist (hereinafter - twice a year), and also undergo the following examinations:
- X-ray examination for tuberculosis - large-frame chest fluorography (in the future - according to indications and once a year);
- a blood test for syphilis (hereinafter - four times a year);
- a blood test for hepatitis B and C (hereinafter - once a year). A blood test for hepatitis B is carried out only in the absence of data on vaccinations against hepatitis B;
- examination of smears for gonorrhea (in the future - twice a year);
- a blood test for HIV infection (further twice a year).
Other diagnostic studies are carried out depending on the identified pathology.
2. Clause 3.4. add the following paragraph:
"If it is not possible to isolate puerperas and newborns for disinfection, agents from the group of surface-active substances (surfactants) can be used: catamine AB, chlorhexidine gluconate, ampholane and other surfactants approved for use in the prescribed manner (nickname - extra M, veltolen, etc. )".
3. From paragraph 2 of clause 4.2.3. exclude words:
"or 0.5% aqueous solution of chlorhexidine gluconate".
4. The first paragraph of paragraph 4.8.7. put in a new edition:
"Medical personnel taking delivery and caring for an HIV-infected puerperal woman and her newborn in the postpartum period must observe personal safety measures (work with gloves during all manipulations, when taking delivery - use goggles or screens)".
5. Clause 4.8.8. put in a new edition:
"The risk of contracting HIV infection arises in emergency situations (cuts and pricks with instruments contaminated with blood and other biological fluids from HIV-infected patients, as well as the ingress of blood and other biological fluids on the mucous membranes of the oropharynx, nose and eyes).
To reduce the likelihood of infection in such cases, it is recommended:
- in preparation for the manipulation of a patient with HIV infection, make sure the integrity of the emergency kit;
- perform manipulations in the presence of a second specialist, who, in the event of a rupture of gloves or a cut, can continue its implementation;
- treat the skin of the nail phalanges with iodine before putting on gloves;
- if contaminated material (blood, amniotic fluid, discharge from the birth canal, etc.) gets on the skin of personnel, treat it with a 70% alcohol solution, wash it with soap and water and re-disinfect it with a 70% alcohol solution. Don't rub!
- if an infected material gets on the mucous membranes of the oropharynx and nose, they are immediately treated with a 0.05% solution of potassium permanganate, the mouth and throat are rinsed with 70% alcohol or a 0.05% solution of potassium permanganate;
- for the treatment of the mucous membrane of the eyes, a solution of potassium permanganate 1: 10000 is used; the solution is prepared ex tempore, for which it is necessary to have a 50 mg sample of the drug, which is dissolved in 10 ml of distilled water;
- in case of injections and cuts, wash gloved hands with running water and soap, remove gloves, squeeze blood out of the wound, wash hands with soap and treat the wound with 5% iodine solution. Don't rub!
In case of accidents, prophylactic administration of thymoside (azidothymidine) 800 mg/day for 30 days is recommended, the drug should be started no later than 24 hours after the accident.
Laboratory examination of persons caught in emergency performed at 3, 6 and 12 months.
6. In Table 1 of Appendix 1:
- in clause 2 "Air and surfaces in rooms" the date of approval guidelines"28.02.96" replace with "28.02.95";
- in clause 9 "Rubber mats", the concentration of chloramine B solution 1.0 is replaced by 5.0 with a disinfection time of 60 minutes. and double wipe.
7. In Table 3 of Annex 1:
- in item 8 "Kuvez" consider the concentration of PVC solution as 0.75;
- in item 10 "Centimeter tape" and item 15 "Grid for graduated bottles" the concentration of chlorhexidine gluconate solution 1.0 is replaced by 0.2;
- paragraph 15 "Enameled basin ..." read paragraph 13, replace the PVCL concentration of 0.5 with 3.0;
- paragraph 17 "Jars for collecting milk, breast pumps, glass funnels" read paragraph 14;
- in clause 17 "Underlaying oilcloths", the concentration of PVC solution 0.25 should be replaced by 3.0 and the concentration of Peroximedom solution 0.25 should be replaced by 3.0 with a disinfection time of 60 minutes;
- Notes to Table 3 shall be supplemented with paragraph 4 of the following content:
"4. When objects are contaminated with blood, disinfection should be carried out in accordance with the recommendations set out in Appendix 3 to the order of the USSR Ministry of Health of July 12, 1989 N 408 "On measures to reduce the incidence of viral hepatitis in the country."

SECTION "QUALIFICATION CHARACTERISTICS OF EMPLOYEES OF STATE ARCHIVES, DOCUMENT STORAGE CENTERS, ARCHIVES OF MUNICIPALITIES, DEPARTMENTS, ORGANIZATIONS, LABORATORIES FOR ENSURING THE SECURITY OF ARCHIVAL DOCUMENTS"

1. The section "Qualification characteristics of the positions of employees of state archives, document storage centers, archives of municipalities, departments, organizations, laboratories for ensuring the safety of archival documents" of the Unified Qualification Directory for the positions of managers, specialists and employees (hereinafter referred to as the CSA section) is intended to address issues related to with regulation labor relations, providing an effective management system for employees of state archives, document storage centers, archives of municipalities, departments, organizations, laboratories for ensuring the safety of archival documents (hereinafter referred to as archives, laboratories).

2. Section CEN consists of three sections: I - " General provisions", II - "Qualification characteristics of positions of employees of state archives, document storage centers, archives of municipalities, departments, organizations", III - "Qualification characteristics of positions of employees of laboratories for ensuring the safety of archival documents".

3. The qualification characteristics contained in this section of the CEN can be used as normative documents or serve as the basis for the development job descriptions containing a specific list of job responsibilities of employees, taking into account the peculiarities of the organization of labor and management, as well as the rights, responsibilities and competence of employees. If necessary, the job responsibilities included in the qualification description of a certain position can be distributed among several performers.

4. The qualification characteristic of each position has three sections: " Job Responsibilities', 'Must know' and 'Qualification requirements'.

The section "Responsibilities" contains the main labor functions that can be entrusted in whole or in part to the employee holding this position, taking into account the technological homogeneity and interconnectedness of the work, allowing for optimal specialization in the positions of employees.

The "Must Know" section contains the basic requirements for an employee with regard to special knowledge, as well as knowledge of legislative and other regulatory legal acts, regulations, instructions and other documents, methods and means that the employee must be able to apply in the performance of official duties.

The "Qualification Requirements" section establishes the level of professional training of the employee necessary for the performance of the prescribed job duties, certified by documents on education, and requirements for work experience.

5. The EKS section does not include the qualification characteristics of the positions of deputy directors of the archive, director (head, head) of the laboratory and heads of departments (heads of departments) of the archive, laboratory. The duties of these employees, the requirements for their knowledge and qualifications are determined by the director of the archive, the director (head, head) of the laboratory based on the qualification characteristics of the respective positions of managers. The requirements for work experience are set 1 year lower than for the corresponding positions of managers.

The distribution of job responsibilities of these employees is established by the internal organizational and administrative documents of the archive, laboratory.

6. In accordance with the qualification characteristics of the positions "chief archivist", "leading archivist" and "archivist", job responsibilities, requirements for knowledge and qualifications of positions " Chief Specialist archive", "leading specialist of the archive" and "specialist of the archive", respectively, not included in this section of the CSA.

7. The qualification characteristics of the positions "archivist", "archaeographer", "paleographer", "methodist", "artist-restorer of archival documents" provide for intra-position qualification categorization within the same position without changing its name. Qualification categories for these positions are established by the head of the archive, laboratory, taking into account the complexity of the work performed or professional skills, the degree of independence and responsibility of the employee in the performance of official duties.

8. In order to improve the organization and increase the efficiency of the work of employees of archives, laboratories, it is possible to expand the range of their duties in comparison with the established corresponding qualification characteristics. In these cases, without changing the job title, the employee may be entrusted with the performance of duties stipulated by the qualification characteristics of other positions that are similar in content to work, equal in complexity, the performance of which does not require a different specialty and qualification.

9. Compliance with the actually performed duties and qualifications of employees of archives, laboratories with the requirements of qualification characteristics is determined by the attestation commission of the archive, laboratory.

10. Employees who do not have special training or work experience established by the qualification requirements, but who have sufficient practical experience and perform the duties assigned to them in a quality and full scope, on the recommendation attestation commission archive, laboratory are appointed to the appropriate positions in the same way as persons with special training and work experience.

II. Qualification characteristics of the positions of employees of state archives, document storage centers, archives of municipalities, departments, organizations

Carries out activities for the effective use of established wage systems, rational spending of the fund wages and timely settlements with archive workers. Takes measures to provide the archive with qualified personnel, rational use and development professional knowledge and experience, systematic professional development of employees, creation of favorable working conditions, compliance with the requirements labor law, production and labor discipline. Organizes in the prescribed manner the certification of employees, makes the appointment, placement of personnel. Supervises work on improving the structure of the archive and the organization of work of employees. It takes measures to introduce rational forms and methods of work into practice, to develop the material and technical base of the archive. Develops schemes for the rational placement of archive documents. Accepts documents for permanent and temporary storage in accordance with the established procedure. Conducts work on targeted examination of the value, accounting and search for undetected cases, draws up the results of these works. Participates in the preparation of materials for the declassification of archival documents. Identifies unique and especially valuable documents. Organizes the accounting of archive documents. Supervises work on checking the availability and physical and chemical condition of documents, preparing and transferring documents for restoration, disinfection, disinfestation, insurance copying and binding. In connection with the release of new regulatory documents of the Ministry of Health of Russia, regulating sanitary and anti-epidemic rules,

I order:

1. To make changes and additions to Appendix 1 "Instructions for the organization and implementation of preventive and anti-epidemic measures in obstetric hospitals" to the order of the Ministry of Health of Russia dated November 26, 1997 N 345 ​​"On improving measures for the prevention of nosocomial infections in obstetric hospitals" (Appendix).

2. The heads of the health authorities of the constituent entities of the Russian Federation and the chief doctors of the centers of state sanitary and epidemiological supervision in the constituent entities of the Russian Federation, the heads of healthcare institutions of federal subordination, accept the changes and additions approved by this order for management and execution.

CHANGES AND ADDITIONS TO
"Instructions for organizing and conducting preventive and
anti-epidemic measures in obstetric hospitals"

1. Paragraph 2.1 shall be stated in a new wording:

"2.1. medical workers employees entering maternity hospitals (departments) undergo a medical examination by a general practitioner, a dermatovenereologist, a dentist, an otolaryngologist (hereinafter - twice a year), and also undergo the following examinations:

X-ray examination for tuberculosis large-frame chest fluorography (in the future - according to indications and once a year);

Blood test for syphilis (in the future - four times a year);

Blood test for hepatitis B and C (hereinafter - once a year). A blood test for hepatitis B is carried out only in the absence of data on vaccinations against hepatitis B;

Examination of smears for gonorrhea (in the future - twice a year);

Blood test for HIV infection (in the future - twice a year).

Other diagnostic studies are carried out depending on the identified pathology.

2. Paragraph 3.4 shall be supplemented with the following paragraph:

"If it is impossible to isolate puerperas and newborns for disinfection, agents from the group of surface-active substances (surfactants) can be used: catamine AB, chlorhexidine gluconate, ampholane and other surfactants approved for use in the prescribed manner (nika-extra M, veltolen, etc. )".

3. From paragraph 2 of clause 4.2.3, delete the words:

"or 0.5% aqueous solution of chlorhexidine gluconate".

4. The first paragraph of clause 4.8.7 shall be reworded:

"Medical personnel who deliver and care for an HIV-infected puerperal and her newborn in the postpartum period must observe personal safety measures (work with gloves during all manipulations, use goggles or screens when taking delivery)".

5. Clause 4.8.8 shall be reworded:

"The risk of contracting HIV infection arises in emergency situations (cuts or injections with instruments contaminated with blood and other biological fluids from HIV-infected patients, as well as the ingress of blood and other biological fluids on the mucous membranes of the oropharynx, nose and eyes).

To reduce the likelihood of infection in such cases, it is recommended:

When preparing for manipulation in a patient with HIV infection, make sure the integrity of the emergency first aid kit;

Perform manipulations in the presence of a second specialist, who, in the event of a rupture of gloves or a cut, can continue to perform it;

Treat the skin of the nail phalanges with iodine before putting on gloves;

If contaminated material (blood, amniotic fluid, discharge from the birth canal, etc.) gets on the skin of personnel, treat it with a 70% alcohol solution, wash it with soap and water, and re-disinfect it with a 70% alcohol solution. Don't rub!

If the infected material gets on the mucous membranes of the oropharynx and nose, they are immediately treated with a 0.05% solution of potassium permanganate, the mouth and throat are rinsed with 70% alcohol or a 0.05% solution of potassium permanganate;

For the treatment of the mucous membrane of the eyes, a solution of potassium permanganate 1: 10000 is used; the solution is prepared ex tempore, for which it is necessary to have a 50 mg sample of the drug, which is dissolved in 10 ml of distilled water;

For injections and cuts, wash gloved hands with running water and soap, remove gloves, squeeze blood out of the wound, wash hands with soap and treat the wound with a 5% iodine solution. Don't rub!

Laboratory examination of persons in an emergency situation is carried out after 3, 6 and 12 months.

6. In Table 1 of Appendix 1:

In paragraph 2 "Indoor air and surfaces" the date of approval of the guidelines "28.02.96" should be replaced with "28.02.95";

In clause 9 "Rubber mats", the concentration of chloramine B solution 1.0 is replaced by 5.0 with a disinfection time of 60 minutes and wiping twice.
Item 17 "Underlay oilcloths" replace the concentration of PVC solution 0.25 with 3.0 and the concentration of peroxymedome solution 0.25 with 3.0 with a disinfection time of 60 minutes;

Notes to table 3 shall be supplemented with paragraph 4 of the following content:

"4. When objects are contaminated with blood, disinfection should be carried out in accordance with the recommendations set out in Appendix 3 to the order of the USSR Ministry of Health of July 12, 1989 N 408 "On measures to reduce the incidence of viral hepatitis in the country."
